Movilidad, mortalidad y saturación hospitalaria: análisis multifuente de la respuesta pandémica diferencial en las entidades federativas de México (2020-2022)
Resumen
Contexto: México registró más de 334,000 defunciones oficiales por COVID-19 entre marzo de 2020 y octubre de 2022, con una distribución territorial profundamente heterogénea entre sus 32 entidades federativas: la tasa de mortalidad registrada en la Ciudad de México (484/100K) fue 11.4 veces superior a la de Chiapas (42.6/100K). Ningún estudio previo ha integrado simultáneamente datos de movilidad, restricciones sanitarias, epidemiología, ocupación hospitalaria, vacunación y pobreza para analizar los determinantes de esta heterogeneidad. Objetivo: Examinar la secuencia asociativa entre política sanitaria (Semáforo Epidemiológico), comportamiento poblacional (movilidad), presión sobre el sistema de salud (ocupación hospitalaria), resultados epidemiológicos (mortalidad) y condiciones estructurales (pobreza), en las 32 entidades federativas durante el período completo de la pandemia. Metodología: Diseño cuantitativo longitudinal integrando ocho fuentes de datos oficiales con más de 350,000 registros pareados. Se aplicaron pruebas de Kruskal-Wallis con ε² como tamaño del efecto, correlaciones cruzadas con desfase temporal y corrección de Bonferroni (α ajustado = .006), test de Mann-Whitney con d de Cohen, correlaciones de Pearson y Spearman con intervalos de confianza al 95% mediante transformación z de Fisher, regresión lineal múltiple por MCO con diagnósticos de VIF y Shapiro-Wilk, test de mediación de Sobel, test de tendencia de Kendall (unilateral) y pruebas de Dickey-Fuller aumentada para estacionariedad. Se construyó un Índice de Respuesta al Semáforo (IRS) por entidad. Resultados: El Semáforo Epidemiológico se asoció con diferencias significativas en movilidad entre sus cuatro niveles (H(3) = 5,976.08, p < .001, ε² = .358) y en hospitalización (H(3) = 7,417.88, p < .001). La movilidad se asoció con las defunciones con un desfase de 21 días (r = -.676, p ajustada < .001, R² = .457), mediado por la ocupación hospitalaria (r = .877 a lag 0, R² = .770). Se documentó fatiga de restricciones: la tercera ola produjo 26 puntos porcentuales menos de reducción de movilidad que la segunda en niveles de alerta comparables (d de Cohen = -3.17, p < .001). La vacunación se asoció con una reducción de la tasa de letalidad por caso de 11.9% a 0.3% (τ de Kendall = -1.000, p unilateral = .008) y del ratio de ocupación UCI sobre hospitalización general de 1.18 a 0.48. La correlación negativa entre pobreza y mortalidad registrada (r = -.616, IC 95% [-.794, -.340], p < .001) se interpretó como sub-hospitalización diferencial; el test de Sobel para mediación a través de la hospitalización no alcanzó significancia estadística (z = -1.55, p = .120), aunque la proporción mediada fue del 21.7%. Un modelo de regresión múltiple ampliado con accesibilidad a carretera pavimentada (CONEVAL) incrementó la varianza explicada de R² = .533 a R² = .636 (F(4,27) = 11.77, p < .001, VIF < 3), con la inaccesibilidad vial como predictor significativo (β = -3.65, p = .010), precisando que el mecanismo operativo de la paradoja es la distancia física al hospital. Conclusiones: La respuesta pandémica en México se caracterizó por una secuencia asociativa de cuatro eslabones (restricciones, movilidad, hospitalización y mortalidad), moderada por la vacunación y condicionada por la desigualdad estructural en infraestructura sanitaria. La evidencia de temporalidad en los desfases y la consistencia de los gradientes sugieren, sin ser concluyentes, una dirección causal que requiere confirmación mediante diseños cuasi-experimentales.
