Factores asociados a la mortalidad en neonatos con síndrome de dificultad respiratoria: estudio de casos y controles en un hospital de segundo nivel del sureste de México
Resumen
Introducción: El síndrome de dificultad respiratoria (SDR) es una de las principales causas de morbimortalidad neonatal, especialmente en recién nacidos prematuros. Aunque diversos factores clínicos y perinatales se han asociado con la mortalidad, existe escasa evidencia sobre los predictores independientes en población mexicana. Objetivo: Identificar los factores asociados con la mortalidad en neonatos con síndrome de dificultad respiratoria atendidos en un hospital de segundo nivel del sureste de México. Material y métodos: Se realizó un estudio observacional, analítico, retrospectivo de casos y controles en el Hospital General de Zona No. 46 del Instituto Mexicano del Seguro Social, Villahermosa, Tabasco, México. Se incluyeron 132 neonatos con diagnóstico de síndrome de dificultad respiratoria, distribuidos en 66 casos (fallecidos) y 66 controles (sobrevivientes), atendidos entre enero de 2022 y diciembre de 2024. Se efectuó análisis descriptivo, bivariado y regresión logística binaria para identificar los factores asociados de manera independiente con la mortalidad neonatal. La magnitud de la asociación se expresó mediante odds ratio (OR) con intervalos de confianza al 95 %. Resultados: En el análisis bivariado se observaron asociaciones significativas entre el requerimiento de oxígeno, los antecedentes maternos, la talla al nacimiento y el número de controles prenatales con la mortalidad neonatal. Tras el ajuste mediante regresión logística binaria, únicamente el puntaje de Apgar (ORa = 0.21; IC95%: 0.05–0.98; p = 0.048) y el requerimiento de oxígeno (ORa = 0.42; IC95%: 0.19–0.93; p = 0.033) conservaron una asociación independiente con la mortalidad. El modelo presentó un adecuado ajuste (Hosmer-Lemeshow, p = 0.584) y una capacidad discriminativa aceptable (AUC = 0.71). Conclusiones: El puntaje de Apgar y el requerimiento de oxígeno fueron los únicos factores asociados de manera independiente con la mortalidad neonatal en pacientes con síndrome de dificultad respiratoria. Estos hallazgos destacan la importancia de la valoración clínica inmediata del recién nacido y del inicio oportuno de las intervenciones respiratorias, además de aportar evidencia local para fortalecer la atención neonatal y orientar futuras investigaciones.
