Meningioma Esfenoorbitario Infiltrativo Grado 3 de la OMS: Revisión de la Literatura, Hallazgos Imagenológicos y Reporte de Caso
Resumen
Los meningiomas esfeno orbitarios constituyen una variante poco frecuente de los meningiomas de la base del cráneo, caracterizados por la infiltración del ala mayor del esfenoides, compromiso orbitario e hiperostosis progresiva. Aunque la mayoría corresponden a tumores grado 1 de la Organización Mundial de la Salud (OMS), las variantes de alto grado representan un hallazgo excepcional y se asocian con comportamiento biológico agresivo, mayor recurrencia y peor pronóstico. Se presenta el caso de una mujer de 45 años con cefalea progresiva de siete años de evolución y proptosis ocular derecha de aparición insidiosa. La tomografía computarizada evidenció una lesión ósea expansiva con hiperostosis del ala mayor del esfenoides y hueso temporal derechos, condicionando reducción del volumen orbitario y desplazamiento del contenido intraorbitario. La resonancia magnética demostró importante engrosamiento óseo con intenso realce dural y edema vasogénico frontotemporal adyacente, planteándose como principales diagnósticos diferenciales un meningioma esfenoorbitario de alto grado y un osteosarcoma. La paciente fue sometida a resección quirúrgica mediante abordaje órbito-cigomático con descompresión orbitaria y apertura del canal óptico. El estudio histopatológico confirmó un meningioma transicional infiltrativo grado 3 de la OMS con compromiso de los márgenes quirúrgicos, indicándose radioterapia adyuvante. Este caso resalta el papel fundamental de la tomografía computarizada y de la resonancia magnética para caracterizar el compromiso óseo, la extensión intraorbitaria y la infiltración dural, elementos indispensables para el diagnóstico diferencial, la planificación quirúrgica y el manejo multidisciplinario de esta infrecuente entidad.
